Transcript Release Form

This signed form must be submitted and on file before any official school documents can be sent from Sacred Heart.

Most applications to colleges, universities, high schools, summer programs, ROTC programs, service academies, NCAA Eligibility Center, scholarship programs, and potential employers, require that an official transcript of your academic history at Sacred Heart Preparatory be forwarded by the school on your behalf.  In order to process any such application or registration, the following statement must be signed and on file at Sacred Heart Schools.

I authorize the release of a copy of my official transcript containing a list of courses I have taken, or attempted, and grades earned, as well as any other educational records, as required or requested by the institutions to which I apply.

I understand that standardized test scores do not appear on my transcript and are not forwarded by Sacred Heart Preparatory, and that I must contact the respective testing service(s) to have my scores sent to any institution.
Sacred Heart Schools considers the electronic signature(s) above equivalent to a handwritten signature.
